After all the wedding excitement, changing your name can seem a bit of a daunting task. I decided to breakdown the name change process, so that you don't feel overwhelmed -- It is super easy!
1st - Update your social security card by visiting your local office with the following documents: 1. You will need to fill out the social security application, noting that you need a corrected card. 2. Form of identification. I took my passport, IL state ID, and birth certificate. 3. Your marriage certificate. https://www.ssa.gov/forms/ss-5.pdf 2nd - Once you have your updated social security card, you can update your state license. There is no application required, just: 1. Your new social security card 2. Marriage Certificate 3. Current State License 4. Piece of mail to prove current residence 3rd - Update all of your information with accounts: bank, credit cards, insurance, travel profiles, email addresses, benefits, etc. All you will need, is your new state license and the marriage certificate. Some will need your old state license as well. 4th - Update your passport. 1. You will need to fill out an application for the name change 2. With #1, you will need to send your old passport, marriage certificate, and new passport photos. It will take 6-8 weeks to get your new passport. If you have updated your passport in the last 12 months, you can use this form: https://travel.state.gov/content/dam/passports/Forms/Form%20Upload%208.1.17/ds5504.pdf If your passport was renewed more than 12 months ago, you can use this form: https://travel.state.gov/content/passports/en/passports/forms.html 5th - Re-apply TSA Pre-check and/or Global Entry. You cannot reapply until you have your update to date passport, so these are the very last items! See you next Tuesday!
